Living Well Counseling is committed to protecting your privacy and providing clear information about the use and limitations of this website. Please review the following policies before using this site.
By accessing or using this website, you acknowledge and agree to the terms described below.
Website Disclaimer
The information provided on this website is intended for general informational and educational purposes only. Nothing on this website is intended to constitute medical, psychological, psychiatric, legal, or other professional advice.
Visiting this website, reading its content, submitting a contact form, sending an email, or otherwise communicating with Living Well Counseling does not establish a therapist-client relationship.
A therapist-client relationship is established only after appropriate intake procedures have been completed, informed consent documents have been reviewed and signed, and both the therapist and client have agreed to begin treatment.
Information provided on this website should not be used as a substitute for individualized evaluation, diagnosis, or treatment from a qualified healthcare professional.
Emergency & Crisis Disclaimer
This website and its contact forms are not monitored continuously and should not be used for emergencies or urgent mental health concerns.
If you are experiencing a mental health or medical emergency, are in immediate danger, or believe you may harm yourself or someone else, contact emergency services or go to the nearest emergency department.
Messages submitted through this website may not receive an immediate response.
Privacy Policy
Living Well Counseling respects the privacy of visitors to this website.
Information you voluntarily provide through a website contact form or email may include your name, email address, telephone number, and other information you choose to share.
This information is used for purposes such as responding to inquiries, determining whether services may be an appropriate fit, and communicating about requested services.
Please avoid submitting highly sensitive medical or mental health information through general website forms or standard email unless specifically instructed to do so through a secure system.
Submitting an inquiry does not guarantee availability, acceptance as a client, or the establishment of a therapist-client relationship.
HIPAA & Electronic Communications
Living Well Counseling takes client confidentiality seriously. However, ordinary website contact forms, email, text messaging, and other electronic communications may have privacy and security limitations.
Once you become a client, additional policies regarding confidentiality, electronic communication, telehealth, recordkeeping, and protected health information will be provided through the practice's informed-consent and privacy documentation.
This website privacy policy is separate from any Notice of Privacy Practices or other HIPAA-related documentation that may apply to established clients.
Telehealth Services
Living Well Counseling provides virtual psychotherapy services to eligible clients located in jurisdictions where the treating clinician is legally authorized to practice.
Amy Logan, MA, NCC, LMHC, LCPC currently provides online therapy to eligible clients located in Illinois and Indiana.
Your physical location at the time of a telehealth session may affect whether services can legally be provided. Clients are responsible for communicating changes in location or residence that could affect eligibility for services.
Information on this website regarding service areas or licensure is subject to change.
Fees, Insurance & Payment
Living Well Counseling is currently a private-pay practice and does not directly accept insurance.
The current standard fee for individual therapy is $160 per session. A limited number of sliding-scale appointments may be available depending on current availability.
If your insurance plan includes out-of-network mental health benefits, documentation may be available for you to submit to your insurance company for possible reimbursement.
Living Well Counseling cannot guarantee that an insurance company will reimburse any portion of the cost of services. Clients are responsible for confirming their individual benefits, coverage requirements, deductibles, reimbursement rates, and other insurance policies directly with their insurer.
Current clients should refer to their signed practice policies and financial agreement for complete information regarding fees, cancellations, payment, and other financial policies.
Good Faith Estimates & The No Surprises Act
Under federal law, healthcare providers are generally required to provide patients who do not have insurance or who are not using insurance with a Good Faith Estimate of the expected cost of non-emergency healthcare services when applicable.
You may request a Good Faith Estimate before scheduling services or at any time during treatment.
A Good Faith Estimate reflects reasonably expected charges based on the information available at the time it is prepared. The actual number, frequency, and duration of therapy sessions can vary depending on your individual needs and decisions regarding treatment.
Additional rights and dispute-resolution options may be available under the federal No Surprises Act if a bill is substantially higher than the applicable Good Faith Estimate.
Professional Credentials & Scope of Practice
Information regarding professional credentials, training, therapeutic approaches, specialties, and areas of practice is provided to help prospective clients understand the services offered by Living Well Counseling.
Descriptions of approaches such as EMDR, Brainspotting, trauma-informed therapy, humanistic therapy, or other therapeutic modalities are provided for informational purposes and do not constitute a guarantee that a particular approach will be appropriate or effective for every person.
Treatment recommendations are individualized after an appropriate clinical assessment.
No Guarantee of Results
Psychotherapy is highly individualized, and outcomes vary from person to person.
Testimonials, descriptions of therapeutic approaches, examples, educational information, or statements regarding potential benefits of therapy should not be interpreted as promises or guarantees of a particular outcome.
Living Well Counseling does not guarantee that therapy will produce any specific result.
